Fees

Chart Copies: $25 fee for producing a copy of the chart up to 20 pages. Every page after that is charged at 0.25 cents per page. Any fees associated with transmission or transportation (ie fax, courier) will be added in addition.
Reports or Notes: Prices start at $25 and will be based on complexity and time taken to complete, charged at a rate comparable to the clinical fee for the time required to complete
Independent or Functional Capacity Exams: Price set in accordance with requested assessment scope, specifics of questions, and reports requested. Fee will be consistent with clinical fees and the duration required to complete; minimum of assessment fee + a report fee
